EMPLOYEE DIES IN AN END LOADER INCIDENT

Investigation abstract

Employee #1 was guiding a concrete pipe using his hands while the pipe was suspe nded by an end loader. The end loader began traveling forward. As the end loader traveled forward, Employee #1 fell to the ground. Employee #1 died from the imp act of the loader rolling over him.
