THE OPERATOR (EMPLOYEE #1) WAS USING A METAL BAR TO CHECK THE SLACK IN THE MANDR ELL. THE BAR SLIPPED, CAUSING THE EMPLOYEE TO CUT HIS HAND WHEN IT WAS STRUCK AG AINST THE SLITTER KNIFE.
