EMPLOYEE FELL FROM PARAPET TO FIRST FLOOR AND DIED
Investigation abstract
Employee #1 was removing forms from the north parapet, connecting the forms to a crane to be taken to the south atrium where a pour was being prepared. Employee #1 crawled over the parapet to a roof level walkway inside the atrium to pull t aper ins out. He was standing on the parapet with a crow bar, prying the form aw ay from the concrete, when he fell from the roof to the first floor, resulting i n his death.
