Event description

Employee killed after falling into tank from fixed ladder

Investigation abstract

Employee #1 was attempting to get onto a fixed pit ladder adjacent to a 12 foot high, 6 foot diameter tank when he fell into the tank. He was killed. At the tim e of the accident, the fixed pit ladder did not extend 3 feet above the floor la nding. A 3-foot ladder extension was available for use, but it had been removed so that parts could be lifted off the tank and out of the 14 ft deep pit.
