Investigation abstract

AN EMPLOYEE WAS SPRAYING SEALER ON A CONCRETE FLOOR. AS HE STEPPED BACKWARD HE T RIPPED ON THE HOSE CARRYING THE SEALANT AND FELL, STRIKING HIS HEAD ON THE CONCR ETE. FATAL INJURIES RESULTED.
