Investigation abstract
EMPLOYEES WERE STANDING ON TRUCK WHILE RAISING THE TRUCK BODY. THE LIMIT SWITCH FAILED AND THE BODY TRAVELED UPWARD PAST THE MAXIMUM SAFE HEIGHT.IT THEN FELL DO WN AND STRUCK THE EMPLOYEES.
