EMPLOYEE WAS FORMING METAL PIECE FOR TOASTER. EMPLOYEE WAS STANDING AT WORK STAT ION IN FRONT OF PRESS. A 45-T A 3 1/2 NIAGRA PRESS # 1820 WAS INVOLVED IN THIS A CCIDENT. EMPLOYEE LOST 3 FINGERS ON RIGHT HAND. THE POSSONS PULL-OUT DEVICE WAS NOT PROPERLY ADJUSTED TO PREVENT OPERATOR'S HAND FROM BEING IN POINT OF OPERATIO N AT TIME OF DIE CLOSURE.
