Investigation abstract

At approximately 3:30 p.m. on August 31, 1984, Employee #1 climbed upon a loaded flatbed trailer in order to get his tie-down chains in place. He fell off the t railer face first and broke his neck when he hit the ground. Employee #1 was kil led.
