FALL FROM OPEN SIDED FLOOR

Investigation abstract

ON JANUARY 1, 1985, AT APPROXIMATELY 1:15 PM, EMPLOYEE #1 WAS STACKING 130 POUND "I" BEAM AT THE OPEN SIDED FLOORS EDGE OF THE 10TH FLOOR. THE GUARDRIAL HAD BEE N TAKEN DOWN SO THE EMPLOYEE COULD GET THE "I" BEAMS CLOSER TO THE FLOORS EDGE F OR LIFTING BY CRANE. WHILE THE EMPLOYEE WAS STACKING THIS "I" BEAM HE FELL OVER THE BUILDINGS EDGE TO HIS DEATH TEN FLOORS.
