EMPLOYEE #1 WAS DRIVING A FORKLIFT TRUCK FROM THE COMPANY YARD TO AN OIL WELL DR ILLING LOCATION. HE LOST CONTROL OF THE FORKLIFT AND RAN OFF THE ROAD. THE FORKL IFT TURNED OVER ON ITS SIDE. THE DRIVER WAS DECAPITATED.
