Investigation abstract
Three employees were rebuilding the wooden portions of a large billboard. They w nd. The injured employee died of electrocution. ere working from 6.1-meter-high (20-foot-high) scaffolds and on 10.7-meter-high (35-foot-high) stringers. There were no guardrails or safety lines protecting th e workers. The employees had not opened the disconnect for the electrical system but were relying instead on a timer and an electric eye. The circuit became ene rgized intermittently either due to a fault or vibration. One employee apparentl y contacted energized parts and received an electric shock.
