EMPLOYEE #1 WAS UNDER THE BED OF A TRAILER, OPENING A VALVE TO RELEASE THE BRAKE S. ANOTHER EMPLOYEE, UNAWARE OF EMPLOYEE #1'S LOCATION, ATTACHED THE TONGUE OF T HE FLAT BED TRAILER TO THE PINTLE HOOK OF A TRUCK AND PULLED AWAY. THE WHEELS OF THE TRAILER ROLLED OVER EMPLOYEE #1'S HEAD, KILLING HIM.
