Investigation abstract

Employee #1 was mechanically installing drywall around ductwork, using the ductw ork as a work platform. He stepped backward, jabbing himself in the back with a sheet metal stiffener. As he lurched forward, he lost his balance and fell off t he ductwork. He bounced several times and landed on a duct transition 12 ft belo w.
