Employee killed in 120 ft fall from platform

Investigation abstract

Employee #1 and coworkers were brush-painting a 100-ton gantry crane. Employee # 1 fell approximately 120 feet from the work platform, and was killed. He had not attached his lanyard to the provided lifeline. No guardrails were provided to p rotect the workers.
