Employee killed in fall from scaffold

Investigation abstract

A plumber's plug blew out of an 8 in. diameter steel pipe that was being tested under pressure. The plug hit Employee #1, knocking him off a 10 foot high scaffo ld. He struck his head on a concrete block wall and was killed.
