EMPLOYEE #1 WAS TORCH CUTTING A TIRE VULCANIZING MACHINE AT THE SOUTH END OF THE SCRAP YARD. HE CUT THROUGH A SECTION WHICH THEN FELL ON HIM, CRUSHING HIM TO DE ATH. THE CAUSAL FACTOR WAS MISJUDGEMENT OF A HAZARDOUS SITUATION
