Event description

Three employees injured by molten metal in mold explosion

Investigation abstract

Employees #1 through #3 and a contract employee were pouring molten quartzite in to a sand mold. After approximately 25 minutes, when the pouring operation was a pproximately three-fourths complete, the mold blew up, spewing molten metal up t o the ceiling, onto the floor, and over Employees #1 through #3. The employees w ere not wearing any protective gear other than safety glasses, and they suffered first-, second- and third-degree burns over their faces, limbs, and bodies.
