EMPLOYEE #1 WAS BUILDING A CONCRETE FORM DECK OVER AN OPEN STAIRWAY. HE LOST HIS FOOTING AND FELL 67 FEET TO THE CONCRETE FLOOR. THERE WAS NO TEMPORARY FLOOR, S AFETY NET, OR LIFELINE AND SAFETY BELT IN USE.
