EMPLOYEE #1 WAS KILLED WHEN A BLAST FURNACE MALFUNCTIONED, SPEWING HOT GASES OVE R HIS ENTIRE BODY. HE WAS WORKING TOO CLOSE TO THE PROCESS AREA. HOWEVER, NO SAF E DISTANCE HAD BEEN SPECIFIED BY THE EMPLOYER.
