Investigation abstract
Employee #1 was standing at the conveyor table taking lumber off the conveyor ta ble. A blade came off an operating chipper located 39 feet away from Employee #1 . The blade knocked the chipper hood off. The hood struck employee #1 in the hea d, killing him.
