Investigation abstract

At 10:40 a.m. on July 29, 2022, Employee #1 was securing roof trusses on the roo f of a three-story apartment complex under construction. Employee #1 removed his hard hat and started to remove the lanyard from his harness. As he removed his lanyard from his harness, he slipped and fell 31 feet to the concrete below, str iking his head on a wooden beam as he fell. Employee #1 was killed by the fall.
