Event description

Employee is electrocuted while changing light fixtures

Investigation abstract

At 4:30 a.m. on August 16, 2022, an employee was removing overhead fluorescent light fixtures and replacing them with motion activated LED fixtures. The employee was working from a scissor lift in the raised position but it was blocking access to an aisle. A coworker was asked to move the scissor lift so he lowered the lift from the ground controls and found the employee unresponsive on the platform. The employee had been electrocuted by contact with live electrical wires.
