Event description

Employee is killed when pinned underneath trailer gate

Investigation abstract

At 10:45 a.m. on October 7, 2022, an employee was about to unload a delivery for a customer. As the employee was lowering the trailer gate, the gate malfunctioned and dropped down to the ground. As the gate was falling, it struck the employee in the head. The employee fell to the ground and became pinned underneath the gate. The employee was killed.
