Event description

Employee Is Killed When Head Crushed By Piling Dropped From

Investigation abstract

At 9:03 a.m. on November 4, 2022, an employee was guiding a forklift operator in a contractor staging yard. The forklift operator was carrying wood pilings that were to be staged for future pile driving operations. The wood pilings were 70 feet long and 2,400 lbs. The employee was in front of the forklift and its load when the piling rolled off the fork and towards the employee. The employee was killed when his head was crushed by the piling.
