Event description

Employee is electrocuted while repairing power lines

Investigation abstract

At 10:15 a.m. on December 9, 2022, an employee was repairing overheard power electrocuted and fell out of the aerial lift as he was not wearing fall lines and was electrocuted. the neutral wire. There were three phase lines in the vicinity of work being done but they were not isolated and tagged as was required, were not tested to verify that they were deenergized, and protective grounds were not installed before conducting the work. The employee was wearing his primary insulating gloves but they had not been tested within 12 months as was required. The employee was attempting to separate the B & C phases of power from the neutral, but the A phase in the vicinity was still energized. The employee was
