EMPLOYEE FALLS, LACERATES AXILLARY ARTERY, LATER DIES

Investigation abstract

At 3:30 p.m. on June 9, 2022, an employee was removing a sheet metal jam from th e T-3 automatic turret punch press machine. While the employee was attempting to clear a jammed piece of sheet metal, the employee stood on top of the working t able to push the piece of sheet metal to a nearby scrap bin. While pushing the s heet metal, the employee miss-stepped and fell from the table. As the employee w as falling, the corner of the sheet metal lacerated the employee under his armpi t. The laceration severed the employee's axillary artery. The employee was hospi talized and later died.
