Employee dies of unknown causes after found unresponsive
Investigation abstract
At 4:34 a.m. on February 28, 2023, an employee working for an equipment and of the work shift when it was time to leave the facility, a coworker tapped the employee on the shoulder in an attempt to wake her up. The employee was unresponsive and had died of an unknown illness. machinery manufacturer was sitting at a lunchroom table. The employee had started her work shift at 5:00 p.m. and worked as a confined space attendant until lunch time, which was from 11:00 p.m. to 11:45 p.m. The lunchroom was used as a staging/resting area for confined space attendants when they weren't performing attendant duties. The employee never returned to work after lunch and was seen going to her car at approximately 12:38 a.m. She returned (scanned in) to the building at 12:51 a.m. When the returned to the lunchroom, she was observed with her arm on the table with her head resting on her arm.
