Event description

Employee dies of a heart attack, is found unresponsive

Investigation abstract

At 9:30 a.m. on April 29, 2023, an employee working as a process technician for cause of death was unknown at the time the narrative was submitted but was later determined to likely have been a heart attack. an oil refinery was in a company break room. The employee did not look well resulting in emergency services to responded, measured the employee's vitals and cleared him to return to work. At approximately 11:30 p.m., the employee was conducting his rounds in Unit #1607. On April 30, 2023, at approximately 12:00 a.m., the employee was heard on the radio. At approximately 3:45 p.m., after the new shift had come on duty, coworkers found the employee in the break room unconscious. A coworker (shift supervisor) began CPR and external EMS was called. EMS services arrived and declared the employee dead at the scene. The
