EMPLOYEE IS KILLED WHEN CRUSHED BY DUMP TRUCK
Investigation abstract
At 7:15 a.m. on May 31, 2023, an employee was inside a Porta-John while a coworker was operating an off road back end dump truck that was traveling across a berm. The right side of the berm gave way and collapsed. This caused the dump truck to slide down the right side of the berm and to roll over onto the Porta-John. The employee was killed.
