Investigation abstract

At 4:45 p.m. on June 22, 2023, an employee was installing a landscape retention wall (2.25-feet high by 12-feet long). A concrete fence post from the residence next door (located at the property line between the construction site and the property east to the construction site), fell to the construction site side, crushing the employee. The employee was killed.
