Employee is killed in fall to ground
Investigation abstract
At 9:00 a.m. on June 27, 2023, an employee was working on top of stacked tobacco barrels at a height of 20 feet, wrapping them in thick opaque plastic to prepare the tobacco for fumigation with aluminum phosphide. The employee unhooked his harness from the cross-arm strap tied to the ceiling beam and attempted to move to another stack of tobacco barrels that was four feet beneath him. There was a 61-inch gap between the barrels, and the employee fell approximately 20 feet to the concrete floor and was killed. The employee died of blunt force trauma to the head.
