Investigation abstract
At 1:40 p.m. on June 29, 2023, an employee was conducting material handling activities. The employee was standing inside the fall radius of the suspended sheet metal (73 inches by 101 inches by 0.25 inches sheet metal weighing 614 pounds), when the plate unexpectedly fell from the rigging clamp.
