Event description

Employee is killed when struck by wrench

Investigation abstract

At 11:00 a.m. on July 17, 2023, a five man servicing crew with Standard Energy rotate back into their original position along with the rod. As the wrenches rotated back, Employee #1 was struck on the side of the head and pushed back into the rig. Employee #1 sustained fractures and contusions to the head and brain. Employee #1 was killed. was employed by Great Western Drilling Company to pull pipe out of a hole. The crew was attempting to pull out the polishing rod, but the polishing rod was stuck, and the crew was unable to pull it out using the servicing rig. Under the advice of the company man from Great Western Drilling, the crew began attempting to back off the rod by rotating it, using two wrenches and pulling on the wrenches using the four floor hand. As the crew pulled on the wrenches, the blockage must have been removed because the rod released and jumped. When the rod jumped, the crew lost their grip on the wrenches, and the wrenches began to
