Event description
Employee is killed when run over by truck
Investigation abstract
At 11:21 a.m. on August 9, 2023, an employee, the company owner, was applying truck wheels. The employee was lying on top of the hose with the hose between his legs. The employee died due to crushing injuries. grass seed around the exterior of cul-de-sac while following to the right rear of a hydroseeding truck. The employee's best friend of 40-plus years was operating the truck and they were finishing up and he was getting ready to back up the truck. The employee usually walked back and forth on both sides of the truck to give the friend a signal. The operator was moving forward approximately 3-5 mph when he lost contact with the employee. The operator immediately stopped, put the truck in park and got out to look for the employee. The operator found the employee lying on the ground where he had been crushed by the
