Employee dies after cuts throat with broken glass
Investigation abstract
At 6:09 p.m. on September 1, 2023, an employee was going to break down a piece of glass to be put into the trash. The glass was 233 pounds and 3/8-inches thick. The employee pulled the glass approximately two feet from the edge of the table and began pushing down on the glass violently. On the second bounce, the glass broke, and, as it gave way, the employee fell down and into/onto the broken glass, severing his trachea and blood vessels. The employee made it outside and was picked up by EMS. The employee died at the ER as a result of bleeding out.
