An employee was pressure washing a vacuum box when she began feeling ill, nauseous, and developed a headache, elevated heart rate, and chills. The employee was hospitalized due to possible benzene exposure. The employee was wearing PPE at the time.

An employee was helping a loader operator transfer catalyst from a bin into a roll-off box when the bin came off the loader's forks and fell on the ground. The employee was standing next to the bin when it fell and attempted to get out of the way, falling down in the process. The employee sustained left and right knee injuries.

An employee was performing maintenance on conveyor lines when he jumped out of the way of a moving rail-guided vehicle (RGV). He landed in the conveyor line and was then bumped by the rail-guided vehicle, causing him to strike an angle iron that was a part of a stationary conveyor. An employee was troubleshooting a hydraulic piston on the wheels of a semi-trailer dump truck. The piston released and caused the trailer to move, crushing the employee's leg. The employee sustained fractures and contusions.

On June 24, 2025, at 3:25 p.m., an employee had just finished dumping his truck at the recycling facility when they took a break in their truck. They began to experience signs of heat-related illness and left the cab of the truck before temporarily losing consciousness and collapsing. The employee was hospitalized due to dehydration, heat exhaustion, and acute kidney distress.
