An employee was removing a re-framed wall stud section on the fourth level of a building. He stepped backward and fell 40 feet down a grease chute to the concrete floor. The employee was hospitalized with swelling and bleeding above his right eye and internal injury to the pancreas.

An employee was walking on a walkway when they fell three stories through an open hole to the ground below. The employee sustained four rib fractures.

On 1/06/2023, an employee was on the fourth floor flat roof of an apartment building construction worksite. The employee was cleaning up nails and trash and putting them in a trash bag. An employee was putting a tarp over a leak in a concrete roof. The employee fell through the roof to the floor 32.5 feet below, suffering a broken collarbone and leg, as well as a spinal injury that required a pin.

An employee was standing on a multi-purpose ladder installing head flashing over a window. Their feet were approximately 6 feet off the ground and three rungs from the top. The employee lost their grip on the drill they were using and the ladder tipped over. The employee fell from the ladder to the ground, resulting in a fractured left wrist, bruised chest, and lacerations to their face and lip.

An employee fell through a hole in the ceiling and suffered injuries to his side and right arm. When he fell, a piece of concrete fell with him and struck his arm. He was hospitalized.

An employee was laying glue on the corner of a three-story commercial flat roof to apply new thermoplastic polyolefin roofing material. He lost his balance and fell at least 35 feet to the concrete ground below. The employee sustained fractures.

An employee was delivering a 65-foot boom lift to a rental customer's construction site. The employee set up the ramp truck in the parking lot adjacent to the construction site. He elevated the boom 4-5 feet to clear a temporary fence. While driving the boom lift off the ramp, the wheels slid to the driver's side of the ramp truck, ultimately sliding off the ramp. The boom lift fell off the side of the truck and rolled onto its side. The employee sustained a fractured clavicle, a dislocated shoulder, separated/fractured ribs, a punctured lung, and a fractured left femur. The employee required surgery.
