At approximately 3:45 PM on 3/4/15, an employee was operating a pneumatic assembly fixture with two handed controls. The fixture's drive cylinder pinched off the tip of his left ring finger. The cylinder pulled off half the employee's fingernail and tissue, leaving the bone.
