An employee was moving a piece of an I-beam for welding when it rolled off the cribbing. The employee went to catch it when it fell and crushed the employee's finger, resulting in a partial amputation.

An employee was helping to disassemble large reels used to store steel wire when a 264-pound flange from the reel fell over onto the employee's left leg. The employee sustained multiple fractures to their leg and ligament injuries to the lower leg.

The injured employee was putting away materials on the warehouse floor as a co-worker was pushing a pallet onto a nearby shelf. The pallet knocked another pallet forward, causing it to fall onto the injured employee's shoulders. The employee suffered fractures in their right and left knees and ankles.

Employees were moving a single man lift into a building and reclining the lift to position it to fit through the door. The lift shifted and fell, causing the employee to sustain fractures to the left tibia and fibula. The employee was hospitalized and had surgery.

An employee was moving a light tower so it could be hitched to a truck. He grabbed the tongue of the trailer hitch on the light tower to slide it to the left. The tongue jack fell off, causing the light tower tongue to drop on the employee's right hand. The employee's middle fingertip was amputated.

An employee was walking down a hallway when they stepped on a small puddle of liquid and fell backward onto their left leg, resulting in a broken left femur.

An employee was on a 10-foot ladder replacing a fan motor on an HVAC system. While removing the guard to facilitate maintenance, the guard slipped. As the employee tired to grab it, their left hand fingers were caught between the belt and pulley resulting in a partial amputation of the little finger and fractures and lacerations to the middle and ring fingers.

An employee was using a fire escape when he slipped on the stairs that were wet due to rain. The employee fell down several stairs, hit his head, and injured his right hand.

Three employees were working on a steam pipe. They had depressurized the line and cut it. As they were preparing to replace the broken pipe section, one of them was struck by pressurized hot water and/or steam, which burned his right arm, chest, and back. He was hospitalized.

An employee was pushing a cart when they stopped to assist someone. As they turned around, they fell backward down the stairs with a box and hand truck landing on top of him. The employee was hospitalized and sustained rib fractures, a hematoma, and a compression fracture/spinal injury.

The injured employee was assisting a Department of Homeland Security (DHS) enforcement operation. During an apprehension, officers returned fire at a subject and struck the employee with a bullet. The employee sustained a gunshot wound to their right hand.

The injured employee was assisting in the disassembly phase after a radar array had been lowered and secured. The crew began removing load bearing pins from an overhead crane to free the radome. A load bearing pin was stuck. The injured employee went to remove the pin manually as a second team member applied pressure from the opposite side. The pin unexpectedly released and struck the injured employee s right thumb, resulting in partial amputation of the distal phalanx including an open distal phalanx fracture and nail bed laceration.

An employee was working on an electrical panel in a mechanical room when an arc flash occurred. The employee suffered burns.

At about 3:10 p.m. on October 3, 2025, an employee was inspecting a car. Two dogs that had been in another car were leashed and tethered to a bollard. As the employee inspected the first car along with a narcotics detection dog, one of the other dogs came loose and attacked the narcotics dog. The employee was separating the dogs when the other dog bit his left ring finger. The last joint of the finger was injured and part of it was bitten off.

An employee was preparing a bundle of green onions for chopping. While holding the bundle in his left hand, he made his first cut using a 9-inch kitchen knife held in his right hand. The knife contacted the tip of his left thumb, resulting in an amputation of approximately 0.5 inches of the thumb that required hospitalization.
