Final narrative
Employee was using a pressure washer when the nozzle became clogged. Employee turned off the machine to change the nozzle, but did not release the stored pressure from the wand. The nozzle blew off, injecting water into his left middle finger.
An employee was replacing a bolt on a scissor lift. A rail fell from the scissor lift and lacerated a finger on his right hand, resulting in the amputation of the fingertip.
An employee was reseating a forklift's counterweight using a second forklift. With the counterweight on the ground, the employee was adjusting it and the support slings. The counterweight started to lean and fell forward, catching the employee's right leg against the first forklift's chassis. Their leg was broken.
An employee was drilling a rivet that held a safety tether to a pin on a scissor lift handrail. The drill bit came into contact with the safety tether, which wrapped tightly around the employee's left index finger and lacerated it.

He elevated the boom 4-5 feet to clear a temporary fence. While driving the boom lift off the ramp, the wheels slid to the driver's side of the ramp truck, ultimately sliding off the ramp. The boom lift fell off the side of the truck and rolled onto its side. The employee sustained a fractured clavicle, a dislocated shoulder, separated/fractured ribs, a punctured lung, and a fractured left femur. The employee required surgery.
