Final narrative
A small crew of workers was working under reactor prefitting beams. They had the beams rigged with a come-along. After getting it into place, they lost control of the rigging and one of the beams fell on the ground and landed on the worker's foot.

An employee was working at elevation, installing fitting support brackets on instrument air piping, when he fell 25 to 30 feet to the ground. He was tied off, but his fall protection disengaged about halfway down, allowing him to hit the ground. He suffered a fractured left forearm, multiple fractured ribs, a skull fracture, and a hematoma/brain bleed.
An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.
