An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.

An employee was using air to blow out the air wash to the blender and grinder station. As the employee reached to verify that the air wash was clean, the unit pinched and amputated their fingertip.

An employee was retrieving a lost drill pipe with a lifting bail when their left thumb got pinched between the table and handle of the lifting bail while trying to re-thread the pipe to lift it out. The employee suffered an amputation to the left thumb.

An employee was removing a core chuck from a stationary roll when their right ring finger was pinched between the chuck and the roll. The employee's fingertip was amputated.

On August 11, 2025, an employee was supervising a vendor install. As an overhead bay door was closing, a pipe wrench fell from overhead and struck the employee's left foot, resulting in an injury that required hospitalization and surgery.

An employee was walking on the job site when they fell to the ground and struck the back of their head on the ground. The employee was hospitalized.

An employee was in an oil pit, changing oil under a vehicle. As she was pushing a safety net covering for the pit out of the way, a small metal bar that held the net came loose and struck the left side of her head. She suffered bleeding and a concussion and was hospitalized.

An employee was cleaning windows on a vehicle when he lost footing and fell approximately 6 to 8 feet down into the pit and contacted metal shelving, resulting in injuries to his neck and the back of his head.

Two employees were changing out a flat tire on a car. The jack holding the car up fell, and the car landed on one of the employees. The employee suffered an abdominal injury and was hospitalized.

An employee slipped while climbing a ladder on a skidder, fell to the ground, and suffered a tibia/fibula fracture. The employee was hospitalized.

An employee was lowering the bucket of a track loader that contained a grade stake and the grade stake punctured his foot, resulting in hospitalization and surgery.

An employee was using a pike to move a pallet of sugar that was stuck in the powder racking. The employee fell backward, landing on his right leg. The employee's leg was fractured and he was hospitalized.

An employee was working to fix the forks on a forklift when the fork(s) fell on her toes. The second and third toes on her right foot were fractured, lacerated, and required stitches.

An employee was delivering a 65-foot boom lift to a rental customer's construction site. The employee set up the ramp truck in the parking lot adjacent to the construction site. He elevated the boom 4-5 feet to clear a temporary fence. While driving the boom lift off the ramp, the wheels slid to the driver's side of the ramp truck, ultimately sliding off the ramp. The boom lift fell off the side of the truck and rolled onto its side. The employee sustained a fractured clavicle, a dislocated shoulder, separated/fractured ribs, a punctured lung, and a fractured left femur. The employee required surgery.
