An employee was operating a box machine. While the employee was removing cardboard knock-out debris from the operator load area, the machine's primary pusher caught her right little finger, causing its amputation at the last joint.

Two maintenance employees were reattaching a butterfly valve between two 6-inch diameter pipes with flanges. The gap between the pipes was not aligning correctly. One employee used a pry bar to lift the flanges while the other employee tried positioning the valve. The pry bar slipped and caused the flanges to fall on the employee's right hand. The employee suffered a crushed right finger and an amputation to the tip of the little finger. The ring finger was later amputated during surgery.

An employee connected a steam line to a hose to clean equipment when the fitting broke loose. They were struck by steam in the left inner thigh, resulting in burns that required hospitalization.

An employee was straining hot water from a pot of rice when the water splashed onto them, resulting in burns to their chest, arms, shoulder, and back.

The employee broke through the partially hardened metal; still-molten metal flowed to the employee's steel-toed right boot and entered through the cloth boot tongue. The employee suffered a third-degree burn to the right foot and was hospitalized.

An employee was cutting extruded aluminum framing material using a horizontal band saw. She went to grab a rag in the machine when the rag contacted the blade and pulled her hand in toward the blade, resulting in a partial amputation of the right index finger.

An employee was working to clear a rotary valve on a pugmill. Two fingertips were amputated by the rotary valve.

An employee was climbing an 8-foot ladder to clean windows on a house. The employee was reaching to the right when they lost their balance and slipped. The employee fell and their foot became caught in the ladder rungs, causing both the employee and the ladder to fall to the ground. The employee sustained a fracture to the right leg requiring hospitalization and surgery.

An employee was offloading material using a forklift when the hoisted material began shifting. The employee went to manually stabilize the load and their right middle fingertip was pinched and amputated.

An employee was packing candles on a line. They reached down to pick up a 12-pack of candles when their right little finger was caught in the conveyor belt, resulting in an amputation at the distal joint.
