An employee was pressure-testing a sand separator in a test booth. The employee entered the test booth to remove a plug on the sand separator. Before entering, he turned off the water hose and turned on the high-pressure pump. As he removed the plug, a failure occurred. The employee sustained a broken arm, and the plug was embedded in his arm.

An employee was sanding a pipe on a training lathe while wearing gloves. The employee's right glove got caught on the pipe and was drawn into the turning pipe, amputating part of his right thumb tip. He was hospitalized.
An employee was installing wood trim surrounding a skylight inside a home. While holding a piece of trim in it's desired location, his pneumatic nail gun discharged, sending a 3/4-inch pin nail into his left forearm.
An employee removed a safety valve on a trailer that unknowingly contained nitrogen gas. The gas released from the trailer and pushed the employee backward off a scaffold approximately 10 feet before they struck a garage door, resulting in a pelvic injury.
