An employee was making rounds to the barns to take care of cattle. The employee was stepping out of a van and the sliding door closed on their little finger. The employee's fingertip was amputated.

An employee was driving a backhoe that was hauling dirt to build a new pond. As he drove between the new pond and an existing pond, the wind blew dirt into his windshield and the backhoe veered off into the existing pond. The bed tipped over but the cab did not. The seat belt caused injuries to the employee's chest including bruised lungs.

An employee was moving cows from one area to another area. When she attempted to close a fence gate, a bull struck her. She was hospitalized with a broken shoulder or collarbone and a possible collapsed lung.

An employee ran into a pipe fence while operating an ATV, suffering head/facial injuries.

The pivot span was elevated when the base beam broke free on one side. This shifted the weight of the span, causing it to roll off the loader on its side. When the span came down, the support rod caught the employee's back as he went to turn and run away from it. It pushed him to the ground. He suffered a back fracture, along with a broken bone in his foot, and was hospitalized.

An employee was walking through the hospital pen and a cow struck him with her head. The employee sustained an orbital fracture.

An employee was working in a holding pen, using a crowd gate to herd cows into milking stalls. She was moving to beat the crowd gate to a manhole, but the gate moved and struck the employee. The employee was hospitalized with two fractured processes in her lower vertebrae.

Three maintenance employees were working on chemical lines feeding into a large above ground wastewater treatment tank. Sodium hydroxide and sulfuric acid reacted in a floor drain and the chemicals sprayed onto the three employees. One employee was taken to the hospital for burns to his eyes.

An employee was removing a pin from a metal gate to let cows out of a parlor. The gate moved and his left middle finger was caught in the pin hole. The employee's finger was partially amputated.

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

An employee was working in the food service warehouse when he experienced chest pain, difficulty breathing, and lost consciousness. The employee sustained carbon monoxide poisoning.

An employee was cutting a piece of metal rod with a metal cutting machine. His right middle finger became caught between the rod and the machine's table. The employee's fingertip was partially amputated.
