Final narrative
On April 22, 2015, an employee was crossing over a grassy area between two facility parking lots on her way to work. As she went to step down to the parking lot, a heavy gust of wind from behind made her lose her balance. She fell down and broke her right leg at the femur.
On November 21, 2016, at approximately 9:00 a.m., an employee slipped and fell on ice on the concrete walking surface, suffering a concussion and neck/back pain. The employee was hospitalized.
On October 7, 2015, an employee operating a floor buffer machine suffered a fingertip amputation when the pinky finger became caught between the machine handle and a door handle.
An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.

As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.
