On May 3, 2019, a forklift was moving pallets when the forks struck and broke a pipe. The injured employee went to examine the broken pipe when a blast of air struck the employee causing a surface burn to their right arm and hand.

An employee was mowing the lawn when the mower caught a silt fence, pulling the stakes out of the ground. A wooden stake then struck his face, causing a burn.

An employee worked with a foreign object inside his boot. After removing the boot at home, the employee found blood and was later sent to the hospital for an infection.

Employee was bleeding the pressure off a weighted ball used to check stormwater pipes for leaks. The weight of the water inside the PVC pipe caused the ball to slide down the pipe. Employee grabbed the rope strap with his right hand and the strap's force and sharp edge severed his right-thumb tip. The finger tip was recovered and delivered to Parkland Hospital, but didn't have enough bone to be reattached.

An employee was walking to their vehicle in the parking lot when they slipped on ice and fell. The employee sustained fractures to their left knee and ankle.

Southwest Research Institute

An employee was walking to their car in the parking lot. Right before getting into their car, the employee stepped on an elevated island curb and slipped and rolled their ankle. The ankle was dislocated and fractured.

APPLIED FOOD BIOTECHNOLOGY, INC.

An employee was using a can seamer in the research and development lab when their left index fingertip got caught between the seam press and the rotating can, resulting in a fingertip amputation.

Ardex Laboratories, Inc.

On January 29, 2024, employee 1 was removing a 55-gallon drum from the stock area when it tipped over on its side. Employee 2 tried to lift up the drum with a forklift, but the drum slipped off the forks and crushed employee 1's right little finger. The fingertip was medically amputated.

Southwest Research Institute

An employee was removing parts from a sonic cleaner, which had a steel metal lid held up using hydraulic shocks. The lid came down on the employee's left little finger, resulting in amputation.

United States Marshals Service

An employee was closing a gun locker in his automobile trunk when his left middle finger was partially amputated by the lid.

An employee was ascending a flight of stairs after exiting their vehicle in an adjacent employee parking lot. The employee stepped to the right to avoid another employee on the left who was descending the stairs. The employee lost their balance and fell on the stairs. They were hospitalized with a laceration/abrasion to their chin, a fractured left humerus, and bruising to their left knee.

U.S. Postal Service

A mail handler was moving BMCs on the loading dock when they fell from the loading dock to the concrete ground below. The employee was hospitalized with head trauma.

SJW Contractors

An employee was on an A-frame ladder while preforming demolition of a ceiling. A section of HVAC ductwork fell on the employee and knocked them to the ground approximately 12 feet below. The employee was hospitalized with a concussion and lacerations.

U.S. Department of Army

On the morning of July 24, 2025, an employee was conducting synthesis of a highly toxic experimental chemical (nerve agent) in a chemical fume hood. The operation was completed at approximately 11:30 AM. The employee was exposed to the chemical and began experiencing symptoms of illness/poisoning around 1:30 PM and was hospitalized.
