On or about May 22, 2015, an employee was cleaning with a sweeper and tripped over the cord and fell. The employee broke a bone in the left arm between the elbow and the wrist and hurt a hip.
An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
