On or about May 16, 2015, an employee operating a mobile pallet jack crushed his big toe between the curb and the pallet jack. The toe was amputated at the hospital.

An employee was driving a forklift to take a pallet for replenishment. She made a turn and the forklift slipped on the concrete floor due to concrete sweating. The forklift struck a rack and the employee's foot was caught against the rack. The employee sustained a fractured heel and required surgery.

As he pulled the bulkhead out of a trailer, its plastic lip caught on the dock plate, causing the employee to fall and twist his left leg, knee, and hip. An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
