On June 5, 2015, at approximately 11:45 a.m., an employee was working with blenders that are used to grind meat. An opening on the bottom of the grinder allows ground meat to fall into a buggy. The employee reached to remove a small portion of meat and his left hand made contact with an energized, rotating auger.

On February 18, 2017, at approximately 5:30 p.m., an employee was replacing the chain on a breading machine conveyor that was turned off. The power was restored to the machine while the employee was checking the tension of the chain. The moving chain caught and amputated the employee's right index fingertip.

On September 6, 2016, at approximately 6:15 p.m., an employee was entering the chicken house to catch chickens. While walking into the chicken house, an overhead bi-fold door, approximately 10' wide by 10' tall, fell and struck the employee on the head, knocking him to the floor. Threads of a nylon strap that held the door in the upright position broke and allowed the 150-pound door to fall. The door continued falling onto his left foot, fracturing his left ankle. He was hospitalized for surgery to repair the fractured ankle.

On May 5, 2015, at approximately 12:30 p.m., an employee stepped onto the side of the box machine to fill the glue pot with glue sticks. The employee's foot slipped off the surface of the machine, causing her to fall to the floor and fracture her hip.

An employee was troubleshooting a thigh deboner on a poultry processing line. As he worked to dislodge a thigh bone caught in the blade mechanism, the machine amputated the first and second digits on his left hand. He was hospitalized.

On September 17, 2025, an employee was cleaning a cutting machine when the cutting blade amputated their right index finger below the first knuckle.

Koch Foods of Mississippi, LLC

An employee was operating a stand-up forklift, picking product for an order. The forklift struck a metal rack and the employee's left arm was caught between the forklift and the rack. He was hospitalized with a fracture to the neck of his radius and an open fracture to the olecranon (elbow).

An employee tripped over a concrete curb stop in a parking lot. The employee fell to the ground, landing on their left hip and breaking it.

Certainteed Gypsum West Virginia, Inc

An employee was conducting routine maintenance on a mixer. New lid pins had been installed, and the employee was rotating by hand to ensure they had been installed correctly. The employee's left middle finger was caught between the lid and top of a pin. The momentum from the mixer continued, causing a crushing injury to the finger. The employee underwent a medical amputation from the top knuckle to the tip of the finger.

An employee was climbing down a ladder when it slipped. The employee fell, suffering a broken scapula and broken ribs. The employee was hospitalized.

An employee was climbing down a 15-foot multipurpose ladder. The employee fell about 8 feet, landing on the concrete floor and the ladder itself. The employee suffered several injuries, all on the right side: broken ribs, a broken sternum, a broken temporal bone, broken shoulder, multiple fractures to the cheekbone, and a brain bleed.
